While microsurgical techniques have already become common knowledge in ENT surgery, a new era has begun with the development of endoscopic surgery. Endoscopy is part of the so-called minimally invasive surgery or key hole surgery. Looking through a key hole, the structures on the opposite side are seen best and not the structures which have already been passed by the tip of the endoscope. One solution to this problem is a biportal approach to the middle ear. The authors show the various approaches to the middle ear, for example, through the tympanic membrane or through the Eustachian tube, which allow safe surgical manipulations.
